Can this insurance be paid on a month-to-month basis or in installments?

Month to month is not an available option, however, you may purchase 3, 6, or 12 months. Please see enrollment form for annual, semi-annual and quarterly cut-off dates.

What methods of payment are available for this insurance?

You may enroll online with a VISA or MasterCard by clicking here. Or you may mail in an enrollment form with a check or money order made payable to STUDENT INSURANCE. You may not pay over the telephone. Never send cash! We are not responsible if you enclose cash in the envelope.

When does my insurance become effective?

Coverage will become effective on the date the insurance company or its representative receives the application and correct premium payment, given that the student will meet the eligibility requirements on the requested effective date.

If I leave school or transfer to another school, can I still keep this insurance?

Yes. Coverage ends the last day of the term chosen and for which premium has been paid, regardless if you transfer or leave school. Please refer to the Eligibility on page 1 of the brochure.

What type of insurance is this and what does it cover?

This is a Health and Accident Insurance Plan. However, Preventive Care Services at no cost to you are now covered under this plan. Our plans are Patient Protection and Affordable Care Act (PPACA) compliant. The insurance benefits are described in the brochure. To view the plan brochure click here. Can I get my money back?

There will be no refunds issued.

Do I have to go to a specific doctor or do you have a list of participating doctors?

Click here to find a provider near you. Please confirm with the provider that they are contracted with United Healthcare Choice Plus as the provider lists are not updated daily.

What if my doctor doesn't take this insurance and I have to pay?

What if I see a doctor but I'm not sick or injured?

Preventive Care Services are covered at 100% with no co-pay or deductible only when the services are received from a Preferred Provider. Please see www.healthcare.gov for complete details of the services provided for specific age and risk groups.

How do I renew my insurance?

You must meet the eligibility requirements each time you purchase coverage.

Coverage does not automatically renew and we do not send out reminders when your coverage will expire.

Will I get an ID Card?

You will receive an ID Card in the mail from United Healthcare two to three weeks after you purchase coverage or you may manage your account online.
